Registration Process
All required forms and documents must be completed and uploaded to SchoolMint HERE. Once you collect all the required documents, registration will take around 30-45 minutes to complete.


Documents you provide (6 total):
1. Child’s Birth Certificate

2. Two Types of Proof of New York State Residency
You must be a New York State resident to attend Zeta K-7 and New York City resident to attend Zeta Pre-K.

3. Immunization Records
All children attending school in New York state are required to receive vaccinations based on their grade level. ​
4. Physical Examination Form
Your child must have a complete physical exam with a licensed healthcare provider before the start of school.

5. Individualized Education Program (IEP) or 504 Plan (if applicable)
​
6. Medical Administration Form (MAF)
​An up to date MAF is required if your child needs any medication (i.e., asthma, allergies, diabetes, etc.) during the school day, and must be completed by you and your child’s licensed healthcare provider.

1. Student Information and Contact Form​
Basic information including guardian(s) details, contact information, special education services required, and medical information.

2. Ethnic Identification Form
The U.S. Department of Education collects and records the ethnic identity of public school Students. This information is used only for statistical analysis, data reporting, and accountability determinations. Children may not be refused admission to a public school because of race, color, creed, national origin, gender, gender identity, pregnancy, immigration/citizenship status, disability, sexual orientation, religion, or ethnicity.

3. Home Language Identification Survey
Identifies your preferred language for communication and whether your child requires additional support to understand, speak, read, and write English. Children may not be discriminated against based on the information provided in this form.

4. Consent to Release Student Records
Provides Zeta access to your child’s records from their prior school.
​
5. Zeta Parent Contract​
Articulates terms of the family-school partnership and our mutual commitment to working in the best interests of our students.
​
6. Media and Photo Release
We plan to document and share our students’ joy by taking photos and videos of our students, families, and staff. These images may be used for staff development, educational materials, or school promotional materials. By signing this form, you are giving your consent for your child and/or your family to be included in these recordings.

7. Zeta FERPA and Directory Information Notice and Consent to Display Student Work
This form and notification refers to the Family Educational Rights and Privacy Act (FERPA) and relates primarily to the display of your child’s schoolwork in our halls and classrooms. The notification provides you with additional details about directory information and accessing your child’s educational records.

8. Zeta Field Trip Consent Form​
Provides parent consent for student participation in our field trips throughout the school year (e.g., trips to cultural institutions, iconic and historic destinations, world-class performances, local businesses, sporting events, parks, educational play spaces).

9. Zeta Computer Usage Agreement
Allows your child to use Zeta computers and Zeta to make accounts for your student on various educational applications.
​
10. Zeta Chromebook Agreement and Technology Fee Form
Outlines expectations for maintenance and usage of Zeta provided Chromebooks. Note that there is an annual technology fee.
